Access to Appearance Properties is available on the Question Properties pane after selecting either the Question Heading or the Question Choice.
The Appearance properties are used to control the display of your survey questions, and change when different Question Types and Question Choices are selected on the Question Properties pane.
Appearance Properties change according to the selected Question Heading and the type of Question.

Input Type
Radio Buttons - The response styles will be preceded by a radio button.
Drop-down Box - All of the responses will be displayed inside of a box that, when previewed or viewed on the Web, will look like:
A drop-down box is used to conserve space visually in the Survey, as all the options are hidden on one line, instead of a line for each response. The text displayed in the drop-down box, by default, can be modified. When a respondent clicks the pull-down arrow on the drop-down box, it will display all the responses for the question.
List Box - This option displays all the responses in a box with a scroll bar if necessary.
Caption - Below the Input Type drop-down box is the Caption drop-down box. Use the Caption drop-down to designate the default text that the respondent will see in the drop-down boxes: either "Click here to choose" or "Click here for choices".
Layout - Use the drop-down box to change the question into the form of a List, Columns, Rows, Table Rows, or a Scale.
Columns - If Columns is selected in Layout, the Column edit box will become activated. Enter the number of columns to be specified.
Hide Question Heading - Hides the question heading.
Randomize Choices - Select check box for this option. The choices will appear in a random order every time the Survey is viewed. This also applies to every time the page is refreshed.
Reverse Order - Selecting Reverse Order will reverse the output of question choices when HTML is created. This allows the Survey to preserve the values that are stored in the database, but reverse the output that is displayed to the user. This can help prevent question bias.

Layout - Use the drop-down box to change the question into the form of a List, Columns, Rows, Table Rows, or a Scale.
Columns - If Columns is selected in Layout, the Column edit box will become activated. Enter the number of columns to be specified.

Fill in the Blank
Hide Question Heading - Hides the question heading.
Show Running Total - This box will become active when the Question Choice has been selected for Display as Slider and Validated to a Whole or Real Number. It will display a Running Total that has been selected by the sliders within the Slider question. This box will display in the final version of the Survey with the specified label preceding it.
Total Label - Determines the label for the Slider Running Total.

Heading
Indicate Odd and Even Rows - If checked, the matrix will have a different background color for even rows than it will for odd rows. The color is specified in the Cascading Style Sheet (survey template) selected for use with the Survey. For more information about Survey Themes consult the section on Survey Themes. To configure which Survey Theme, view the section regarding the Format Pane.
Repeat Heading Every ___ Rows - This option will display the headings of the survey again after the specified amount of rows. This can prove to be useful for matrix questions with a large amount of topics; if this is true, the headings may not be visible on-screen when a user is responding to one of the later topics.
Hide Question Heading - Hides the question heading.
Randomize Table Rows - Matrix rows will appear in a random order each time. This also applies to every time the page is refreshed.
Advanced - This link will take you to the Table Properties Dialog, where you can modify Borders, Layout, and Accessibility.
Matrix Choose One Label
Input Type - To format the Input Type for a Matrix question, first click the "+" next to the desired Matrix Question's heading, and then choose each side of the matrix independently for formatting in order to view the Input Type box. Radio Buttons or Dropdown Boxes can be selected for input types.

Fill in the Blank
Alignment - Sets the location of the cursor inside the text box when the Survey is taken by a respondent. Potential options are Left (the default value), Center, Right.
Symbol - This option can be used to display a symbol ($, %, EA) next to a Fill in the Blank field.
Caption - Use the Caption drop-down to designate the default text that the respondent will see in the drop-down boxes: either "Click here to choose" or "Click here for choices".
Slider - Only available when the Fill in the Blank questions has been validated to be a Whole or Real Number. This formats the Fill in the Blank question so that the data input uses a Slider. A small box will contain the numerical value that the current position of the slider represents.
Note: A minimum and maximum value must be assigned to represent the range for the slider. The example above uses values of 0 and 10.
Step - Define the numerical value used to separate each position on the Slider.
