Below are about a third of the Church Health questions from the comprehensive Ministry Management questionnaire.

The entire Ministry Management questionnaire is about 100 questions; the Health questionnaire contains about 65 questions. Both are formatted in a way that enables the congregation to complete the questionnaire in about 10 minutes. This means you can do it in a worship service, which will give you the most useful response.
